Oregon Medical Group is pleased to provide a new, electronic system for medical records requests. You can complete requests from any smart device, phone or computer — anytime from anywhere — for a fee of $6.50. Your record will be delivered right to you, either by mail or electronic delivery.
To request your medical record, click the “Request Record” button below. This will take you to a portal called Ciox. Complete the form, review, sign and submit the request. Records can be delivered right to you by a PIN-protected email or they can be mailed to the address you enter in the request.
